In order to create natural looking lips, the physician must follow certain rules:

Use only temporary fillers, such as Restylane/Juviderm.
Use moderation in the amount of filler.
Avoid augmenting the upper lip if the bony maxilla is prominent.
The filler should preserve the “M” shape of the upper lip.
The lower lip should be larger than the upper lip.
Avoid filling the lower lip if the chin is weak.
If the lips are wide, the filler should be injected more towards the center of the lips, and avoid the sides.
Sometimes, all you need is to fill the groove just next to the vermilion.
Filler must be injected on sterile surfaces. No exceptions.
Patient should avoid lipstick for 1 day after treatment.

Temple – One of the neglected areas of attention is the temple area, which loses volume and needs treatment to provide it a youthful look. The options are hyaluronic acid or fat injection.
Forehead – For the forehead, glabella (angry lines), and crowfeet (smile lines), the best treatment for wrinkles is Botox or Dysport. Fillers should not be used due to the risk of serious complications.
Cheeks – Cheeks should not be augmented if the chin is weak or if the lateral eyebrow is low.
Upper Lids – For the upper lids, fillers look unnatural. The best approach is surgery.
Lower Lids – If a patient has “bags” under their eyes, they need surgery. If there are depressions in the lower lids, then fat injection is the best option.
Narrow Face – Augmentation of the cheeks should be at the lateral submalar area.

Chemical Peel vs Fillers

I have been asked many times what is the preferred choice to improve facial wrinkles, chemical peel or fillers. Deep chemical peels (or phenol peels) are no longer popular due to the unnatural look it provides. The medium peel can improve the skin texture, reduce pigmentation and age spots, and reverse sun damage. Every woman over 30 can benefit from a peel.

On the other hand, fillers are used to fill! Although some physicians use fillers (such as Restylane, Perlane, Juviderm, Surgiderm) in the Crowfeet (smile lines), Glabella (angry lines), or Forehead, patients must know that it carries major risks such as skin necrosis. Therefore, wrinkles in the forehead and around the eyes are best treated with Botox or Dysport. When there is a need to fill flat cheeks, the best choice is fat injection. For deep nasolabial folds, Radiesse is the preferred approach. For wrinkles around the mouth, the best approach is using Restylane, Juviderm, or Surgiderm.

Botox and Dysport are substances that paralyze muscle. Therefore, it is imperative for a physician to have knowledge of muscle location and function prior to injection. Many physicians that inject Botox are not familiar with the exact locations of the muscles in the face.
