<p>The changes that occur with aging are a loss of volume, gravity changes, loss of skin elasticity, and the formation of wrinkles. The purpose of rejuvenation is to restore and correct these changes.</p>
<p>I will start with correcting the loss of volume. With aging, the malar and submalar area, and the nasojular area lose volume. Also, the nasolabial folds (groove) and marionette grooves become deeper. The lips lose volume and become thinner. Corrections of these area require fillers. Although many clinicians prefer the artificial fillers such as hyaluronic acid, radiesse, sculptra and other temporary measures, I personally prefer the fat transplant for different reasons. A fat transplant has the ability to fill large volumes, it is natural (the patient&#8217;s own fat), and most importantly it is long lasting. Restoring volume to the face requires a deep understanding of the concept of beauty, and the determination of the exact location and amount of volume loss. A picture of the patient at 20-30 years old can be used as a reference.</p>
<p>It is important to note that volume replacement becomes volume addition in young individuals. For young individuals, an artistic eye in a clinician takes a prominent role in achieving optimal results in beauty enhancement. Creation of high cheek bones will result in a youthful and attractive look in flat cheeked individuals. One should avoid adding laterally to the cheeks in wide-faced individuals, but in thin-faced individuals it will enhance the look. In others, the submalar area needs to be addressed. The fat could improve deep nasolabial, nasojular and marionette grooves as well as the pre-jowel area. The success of fat transplant is dependent on technique and may last for years.</p>
<p>The correction of gravity changes is best done by surgical procedures such as face lifting, although some improvement can be achieved with fillers.</p>
<p>The loss of skin elasticity can be improved by chemical and laser peels as well as with ammonium lactate products. The degree of skin elasticity loss is individual and is genetically determined.</p>
<p>The correction of wrinkles is done with different approaches. In the upper part of the face (i.e. forehead and periocular (eye) area) the treatment of choice is the botox injection. Although the effect only lasts several months, the improvement in wrinkles is accumulative in subsequent botox treatments so that the patient can expect long term improvement in wrinkles that appear at rest. For this part of the face, the preferred approach for wrinkles is the use of fillers such as restylane, silicone, and artefill.</p>
<p>The most common complications with injections are bruising and swelling.</p>

<p>There is great confusion today among patients seeking facial aesthetic rejuvenation.</p>
<p>The final outcome of this process depends mostly on what type of cosmetic doctor they meet for consultation. For example, a woman who can benefit the most from a facelift, if she first seed a Dermatologist, will be advised to have a filler in order to look younger and attractive, because a Dermatologist is not trained to perform a facelift. In order for the Dermatologist to keep that patient, he will advice a treatment plan that will include: Botox injections, Restylane, Juvederm Radiesse, and other fillers, chemical peels, as well as LASERs. Women in their 30&#8242;s-40&#8242;s might benefit from such approach; however, as these women age and develop skin laxity and jowls, fillers alone will not adequately rejuvenate her, for several reasons. A youthful face has a triangular shape with the chin serving as the apex of the triangle. Whereas, the aging face has more of rectangular shape.</p>
<p>It is apparent that, although filler application to the nasolabial folds and marionetters lines might improve them, fillers alone will not convert the rectangular aging look to the more triangular youthful look. another reason for having a surgical procedure, instead of just fillers, is that usually at this stage, the neck begins to show laxity. No reliable treatment have improved platysma neck bands, has little support.</p>
<p>On the other hand, many Dermatologists offer deep chemical peels to patients with facial and neck wrinkles, promising them the same results as a face lift, without surgery. It is impossible to achieve a lift by peel alone. the small life effect of the jowls, after a deep chemical peel, is temporary. Following a deep chemical peel, the facial skin looses its pigmentation, without improving the deep lines, leaving the neck laxity and bands that need to be corrected. The patients, undergoing a deep chemical peel, look unnatural with the face looks much younger then the neck. There is also a sharp demarcation line along the patient&#8217;s jaw line, with a pale white color of the face compared to the natural pigmentation of their neck.</p>
<p>Fillers have a place for young women without significant facial skin or muscle laxity. On the other hand, if the patient goes first to a Plastic Surgeon, she usually will be recommended to have facial surgery if she has skin and muscle laxity, and at the same time to fillers, Botox and chemical peels. In this was, the patient will obtain the optimal results she is seeking and the surgeon can meet her expectations. Today, surgery of the face is moving towards a more conservative approach with minimal incisions, a quick return to work, and long lasting natural results.</p>
<p>With regard to potential complications, they might appear with surgery as well as with fillers. So this is not a reason to prefer one over the other. It is advisable for prospective patients to have a consultation with a Plastic Surgeon who has the knowledge and expertise to offer them the optimal results.</p>

<p>In recent years, cosmetic enhancement has been growing in popularity. Nearly 11 million surgical and non-surgical cosmetic procedures were preformed in the United States alone; 1 million of these procedures were preformed on men. The incidence of non-surgical procedures increases every year at a much higher rate than surgical procedures. This has led to a more conservative approach in surgical rejuvenation of the face.</p>
<p>Although non-surgical procedures, such as filler and injectables, are appropriate for younger patients with early signs of aging, it cannot replace surgery for more advanced signs of aging. Changes in the face are due to the long term effects of gravity, loss of volume (in fat, muscle and bone), and changes in the skin due to muscle activity and loss of elasticity. After age 20, the skin loses 1%-2% of elastic fibers each year. In order to achieve optimal results, there is a need for a combined approach of surgery first and after recovery, additional non-surgical procedures, such as fillers, injectables, or chemical peels.</p>
<p>The goal of cosmetic rejuvenation is the restoration of volume, and symmetry. Each part of the face must be proportional and in harmony. A youthful look requires a triangular shape with the chin as the apex of the face. When the jowels become prominent with age, converting the triangular youthful look into a rectangular shape, the only way to restpre this change is through surgical procedure. For example, in regards to proportion, a longer nose will sometimes need shortening for facial balance and harmony.</p>
<p><strong>FILLERS</strong></p>
<p>Fillers are used to fill depressions in the forehead, nasojugular groove, scars, nasolabial folds, marionette lines, and for augmenting the cheeks, jaws lines and chin. There are two groups of fillers: one is biodegradable, the other is permanent.</p>
<p>The bio degradable fillers are temporary and may last up to one year. There are many types of fillers on the market that are used in Europe and other countries, but only a few of them have been tested for safety and approved by the FDA. The most common are hyaluronic acid derevities that are monologous in structure, meaning it is a substance all ready present in the body. therefore, there is no need for allergy testing. Those approved by the FDA, such as Restylane, Perlane, Hylaform and Juvederm are being extensivily used. They have completely replaced collagen. Collagen required allergy testing and the results were extremely short lived.</p>
<p>The permanent fillers include Silicone, Gore-tex and Artefill. Since they are foreign material they can carry the risk of reaction with granuloma formation.</p>
<p>There is an intermediate group of fillers that include Radiesse and Sculptra that posses a slower process of degration and therefore last longer then a year. Another alternative is the Fat Graft that has recently become more popular since a larger volume of replacement can be preformed. If this procedure is done correctly, it may last for many years.</p>
<p><strong>EVALUATION</strong></p>
<p>When a Physician (Cosmetic Surgeon or Dermatologies) evaluates a candidate for cosmetic enhancement, the fitst step is to obtain a detailed medical history, including allergies and medications. It is crucial that the patient refrain from medications such as aspirin, Advil, Vitamin , Omega 3, Coumadin, and other drugs that affect blood coagulation for at least 1 week prior to treatment. It is necessary to rule out infection in the area to be injected and other parts of the body (for permanent fillers).</p>
<p>Next to take place, is the aesthetic evaluation to fully address the patient&#8217;s concerns. The area to be injected needs to be marked and shown to the patient before the injection. Pre-injection photographs are mandatory. The plan of treatment will address improving the signs of aging, or improving the aesthetic look by correcting proportions and restoring harmony and balance to the face, or both.</p>
<p>In my analysis, I personally prefer to first show the patient what he or she needs to look his or her best (whether surgical or non-surgical treatments) and then to  carefully listen to the patient&#8217;s concerns. If I feel that the treatment for the patient&#8217;s concerns will provide minimal change, I discourage the patient fro proceeding forward with the treatments. As an expert, it is my responsibility to offer the right procedures and the right filler or injectable to my patients.</p>
